Claims reminder:

- Claim may be subject for denial if damaged had been repaired without prior approval from Intrastrata.

- As stated in your policy, in case insured unit is wreck or non-operational, you are required to make arrangements for safekeeping of the insured unit to IntraStrata.

- For easy and instant filling of claims,we suggest that photocopy of all first five claims requirement stated above should have a copy in your car.


For Bodily Injury

• Police Report/accident investigation report/affidavit of loss of witness

• Medical Certificate

• accident Receipts of medicines/hospital bills

• Birth Certificate (if minor/incapacitated)

• Certification from school/company (for Group Insurance)



For Medical Reimbursement Claim

• Charge slip/prescriptions/statement of Account

• Official Receipts of medicines purchases related to accident will be reimbursed.



For Death Claim

• Police Report/accident investigation report/affidavit of loss of witness

• Death Certificate

• Funeral/Burial Receipts

• Medical Certificate (if minor or single)

• Marriage Certificate (if married)

• Certificate from school/company (for Group Insurance)



Additional Requirements in case of Passenger Vessel Claim

• Passenger Manifest

• Sworn statement/Marine protest of the master of the vessel

• MARINA Certificate of Inspection

• Clearance from Philippine Coastguard (if applicable)

General Requirements

a. Insurance Policy and its Official Receipt

c. Certification from Bureau of Fire Protection

d. Photographs

e. In the absence of CTC, submit Assured's Affidavit or Statement declaring nature of ownership of the property

f. Location of property

g. Extent of damage and salvage value

h. Value of property damaged or destroyed

i. Liens and encumbrances

j. Year the building was constructed and original construction cost (for building loss only)

k. Provide any information relative to the loss.



Additional Requirements for Building Loss

a. A builder's or architects estimate based on plan and bill of material showing the cost of placing the building or improvements in the same state or repair as before the fire.

b. Certiifed copy of the building permit

c. Certified copy of declaration of Real property

d. Copy of plan of the building or sketch prepared by an Engineer/Architect

e. Lease contract or agreement if the assured is not the lot owner

f. Certified lot title

g. If purchased, submit Deed of Sale



Contents / FFF / Machinery & Equipment Loss

a. Contract of Lease or Agreement (if Assured is not the building owner)

b. A complete inventory of all the items contained in the insured premises showing the cost price of each and date acquired.

c. A detailed inventory or list of the articles damaged or destroyed showing the cost price of each and date acquired.

d. Records, purchase invoices, delivery receipts or any other proof of acquisition



If property is owned by the company or corporation:

a. Mayor's Permit/Business License/Certificate of Registration of Business Name

b. Certified copies of Income Tax Returns for the last two (2) years and accompanying Financial Statements filed with BIR, in case of total loss.



Stock-In-Trade

a. Mayor's Permit/Business License/Certificate of Registration of Business Name



In Case of Total Loss

a. Certified copies of Income Tax Returns for the last two (2) years with accompanying Financial Statements filed in BIR

b. Purchase and sales invoices/Delivery Receipts or vouchers or Official Receipts (Purchase and Sales) / Purchase and sales journal and other business records.

c. In the absence of invoices, submit supplier's certificate of itemized sales and deliveries

d. Latest inventory of merchandise submitted to BIR prior to the loss

e. Other Pertinent documents relative to the business



Incase of Partial Loss

a. Certified copies of Income Tax Returns for the last two (2) years with accompanying Financial Statements filed in BIR

b. Purchase and sales invoices/Delivery Receipts or vouchers or Official Receipts (Purchase and Sales) / Purchase and sales journal and other business records.

c. Complete inventory or list of all the damage items contained in the Insured premises showing the cost price of each, extent of loss,and salvage value if any.

d. Other pertinent document relative to the business



*Very Important Reminders

a. If you have other insurance from other companies covering the same properties with claim (especially stocks-in-trade, good in process and inventories), immediately notify Intrastra Assurance Corporation.

b. If there be any alteration, changes or modifications in the occupancy and construction of the insured property, please notify Intrastra Assurance Corporation immediately
